Life Insurance
Payable for death from any cause, except suicide within two years after the
effective date of inception of coverage, or an increase in coverage resulting
from a change in schedule, a change in earnings basis, or a change in class.
Accidental Death and Dismemberment Insurance
Loss must occur as a result of an accident while insured and within 90 days
after the injury is sustained. Losses not covered are those resulting from
war, act of war, suicide, attempt at suicide, bodily or mental infirmity or
disease, any bacterial or viral infection (unless the infection was the result
of an accidental injury or bacterial infection which results from the accidental
ingestion of contaminated substances), or travel in an aircraft except as a
passenger without duties aboard.

insurance contract issued pursuant to such Plan and held by the Trustee, contain

is inconsistent in any respect with such provisions. Although the certificates
of insurance or riders, as applicable, provided for insured individuals describe
the individual's benefits more fully, the extent of the insurance for each
individual is governed at all times by the complete terms of the master policy.
Auditing Provision - Prudential reserves the right to conduct audits to verify
the number of eligible employees and to verify that the insurance benefits
provided are supported by each individual's salary or position.
The Certificate of Insurance that insureds receives describes the benefits and other provisions more fully. The complete terms of the master Group Insurance Policy governs at all times.
