Non-Participating Firms
Non-Participating Firms: To request an enrollment form for your firm's participation in the AICPA Group Insurance Plan simply call the Plan Agent, Aon Insurance Services, at 1-800-223-7473.
Participating Firms
Participating Firms: Enrollment forms must be submitted for new employees requesting coverage and/or if the firm increases to a higher schedule maximum. To request required forms for the AICPA Group Insurance Plan, please call the Plan Agent, Aon Insurance Services, at 1-800-223-7473.
Firms with:
| • | Less than 25 eligible lives (excluding proprietor/partners) are required to complete a Group Insurance Medical Statement form |
| • | 25 or more eligible lives (excluding proprietor/partners) are required to complete a Request for Group Insurance Plan Coverage form |
